This notice will tell you about the ways in which Hospice may use and disclose medical information about you. This notice also describe your rights and certain obligations Hospice has regarding the use and disclosure of medical information. Within fifteen working days following receipt of a complaint, Hospice of Michigan will deliver to the complainant a written report of the results of the investigation.
